Approved School Forms

The following forms are intended to be used by EAB-approved schools. Schools seeking initial approval should contact the EAB before using any of the forms.

A brief description of each form, which can be downloaded as either a PDF or Microsoft Word document, is provided below. Users have the option of completing the Word version of each form electronically.

Special Note: The EAB is currently in the process of standardizing its forms as part of a comprehensive revision of the School and Program Approval Guide. All of the following forms have been recently updated with the exception of the Change of Ownership and School Personnel Information forms.

Institutional Plan

Beginning with the 2010 renewal process, all schools will be required to submit an institutional planning document.

Change of Ownership

If you do not file a change of ownership application within 10 business days after the legal transfer, the school's EAB approval ends and you have to start the application process over again.

Surety Bond

A surety bond ensures that students can recoup tuition and other costs if the school closes. Make sure that your surety company also attaches the power of attorney for your bond.

New or Substantially Revised Program Approval Application

Wis. Stats. §38.50(7) requires all private postsecondary schools not otherwise exempt to obtain approval from the Educational Approval Board before advertising or doing business in Wisconsin.

Application to Solicit Students in Wisconsin

To recruit prospective students in Wisconsin, you need to fill out this application, pay a $200 fee, and designate coverage of $2,000 in the general school surety bond.
